Established in 1980, Cumberland Valley Rheumatology has been providing rheumatology specialty care for 40 years. The practice includes one physician, board certified in Rheumatology and a nurse practitioner certified in family and adult medicine. We are pleased to announce that we have launched an online Patient Portal in our office. With the Patient Portal you can: View Personal Information, View Your Health Record, Receive Educational Materials, Receive Appointment Reminders, Request Prescription Refills

Our Patient Portal will allow you to manage your health care from any location with internet access. Because your privacy is important to us, we require sign up for the portal in our medical office with a photo ID.

Your Responsibility

Payment 
You must pay any  copay and deductible amount
or any amount  that you know is not covered  by your insurance at the time you receive the service.
Payment will be collected at time of check-in.   


If a payment plan is necessary, please call us to discuss options for interest free payment plan
PRIOR to your appointment (minimum balance $100). Call our office at 717-267-2065.

Referrals
We ask our patients to be familiar with your insurance coverage. If referrals are required by your insurance policy, it is your responsibility to arrange for a current referral to be in place for your office visit. In order to avoid any complications or misunderstandings, we ask that you arrange for a hard copy of the referral or a phone call from your referring doctor's office to be forwarded to our office prior to your visit. 

Patient Appointment Reminders
Patients please bring a copy of all medications you are taking to your appointment. If for any reason you cannot make your appointment with us, please contact our office a minimum of 24 hours (2 Business Days) before your scheduled appointment as a courtesy to us and to others waiting to be seen. If a patient "No Shows" for an appointment, the patient will be billed a fee of $25 for the missed office visit. This is not a billable charge to the patient's insurance company, it is the personal responsibility of the patient.
