Emergency Care
If you feel as though your condition is life threatening, contact 911 immediately for transport to the nearest hospital or emergency clinic.
In the event you go to the hospital emergency room, please notify our office as soon as you are available and let us know of your visit, so we can update your medical record for our Provider.
Items to Bring With You
To ensure that your visit to our office goes as smoothly as possible, please bring the following items you will need to have when you arrive.
Please bring the following:
- Questions to Ask – Please feel free to bring a list of questions you would like our Provider to answer.
- Any pertinent medical information, records, or test results given to you by other physician offices.
- Your health insurance card (s), and any required authorization, including Rx insurance card, if applicable.
- A list of any medications you have been taking at home and/or the medicines themselves in their prescription containers.
- A list of any known food and/or drug allergies.
- Co-Payments or co-insurance are expected at the time of service and (cash, personal check, VISA, MasterCard) are accepted.
Referrals
If your insurance requires a referral, it is your responsibility to obtain one prior to your appointment from your primary care physician.
Failure to do so may result in your appointment being rescheduled and you being responsible for your bill in full. Please call our office at 717-267-2065 prior to your appointment to ensure we have a copy of your referral on file.
Prescriptions
Cumberland Valley Rheumatology, P.C. utilizes Surescripts for e-prescribing, a web based solution that exchanges prescription information electronically to the pharmacy.
Please allow our office 72 hours (3 business days) to accommodate your prescription refill requests. Prescription refills for controlled substance medications will only be refilled during your medical visit by one of our treating physicians.
APPOINTMENT NO SHOW POLICY
We understand that you may not be able to keep all of your scheduled appointments. Please understand that missed appointments have a detrimental impact on our practice, not only financially, but they also affect our ability to serve other patients in need of medical care. We understand there may be inclement weather or other circumstances that require you to cancel your appointment. If you must cancel your appointment please do so at least 24 hours in advance of your scheduled appointment so that we can schedule another patient in your appointment slot.
Patients who are unable to keep their scheduled appointment must call the office one business day (24 hours) prior to your scheduled office visit. If a patient “No-Shows” (does not call or notify the office of their cancellation 24 hours prior to your scheduled appointment) you will be billed a no-show fee of $25 for an office visit. This is not a billable charge to the patient’s insurance company; it is the personal responsibility of the patient.
If you "No-Show" more than two appointments in a row we may discharge you from the practice.
If you do not keep (no-show) your initial appointment you will not be rescheduled unless your primary care physician contacts our office to request approval from our physician to schedule another appointment for you.
