If your insurance requires a referral, it is your responsibility to obtain one prior to your appointment from your primary care physician.

APPOINTMENT NO SHOW POLICY
We understand that you may not be able to keep all of your scheduled appointments. Please understand that missed appointments have a detrimental impact on our practice, not only financially, but they also affect our ability to serve other patients in need of medical care. We understand there may be inclement weather or other circumstances that require you to cancel your appointment. If you must cancel your appointment please do so at least 24 hours in advance of your scheduled appointment so that we can schedule another patient in your appointment slot.
Patients who are unable to keep their scheduled appointment must call the office one business day (24 hours) prior to your scheduled office visit. If a patient “No-Shows” (does not call or notify the office of their cancellation 24 hours prior to your scheduled appointment) you will be billed a no-show fee of $50 for an office visit. This is not a billable charge to the patient’s insurance company; it is the personal responsibility of the patient.
If you "No-Show" more than two appointments in a row we may discharge you from the practice.
If you do not keep (no-show) your initial appointment you will not be rescheduled unless your primary care physician contacts our office to request approval from our physician to schedule another appointment for you.
